Admission:

Once families have made the decision to waitlist at Darley House, we require the following documentation;

• Copy of Aged Care Client Record (ACCR)
• Letter from Centrelink or DVA regarding Assest Assessment
• Application for Permanent Entry to an Aged Care Home

All nursing care delivered by Registered Division 1 Nurses and EN Nurses and Patient Care Workers.
