CHOICES AT HOME
Our range of services are designed to enhance the wellness and quality of life of older people, people with disability and carers who are living independently in their own homes.
Our Services
Telecare offers peace of mind if you are living independently at home. Each morning you will receive a phone call to make sure you are well and to have a quick chat.
Community Visiting is available for residents of eligible aged care facilities. You and our registered volunteer can chat, share a cup of tea or a fun activity.
Friendly Visiting is a relaxed and social experience. You and our registered volunteer can chat, share a cup of tea, play a board game or take a short walk around your home and gardens.
Shopping Service can assist you with your grocery shopping, bill paying and visiting the bank or chemist. We'll even unpack your groceries with you!
Garden Maintenance includes lawn mowing and light gardening services.
Community Restaurant provides lunch at a variety of local venues. Enjoy the opportunity for a meal out with friends. Transport can be arranged.
Meals on Wheels offers a large selection of inexpensive frozen meals available for pick up or delivery five days per week Monday to Friday.
Group Activities provide day trips and a variety of social support activities.
Access Our Services
Follow these steps to register and access our services:
1. Phone My Aged Care on 1800 200 422, (if you are over 65 years)
or
Phone NDIS on 1800 800 110, (if you are under 65 years living with disability)
2. Request a referral to Choices at Home for the service you wish to receive
3. Complete an assessment
4. Choices at Home will receive your referral and make contact with you to arrange service delivery
